I found it: just go to http://gpnethelp.com/ and fill in two out of the three fields. Then click on locate account. An advertisement will appear. Scroll down below it and click on No, thanks, and blah blah blah. Then, on the page that appears, click on Submit and you're all set! A notification email should be received!

What is the Member ID? It's the Bill ID that is written in the message you received when you subscribed.
