Création d'un formulaire

bonjour,
voici mon problème, je souhaite crée un formulaire sous WORD 2010. j'ai afficher l'onglet "développeur" j'ai bien activé la fonction "création" mais quand je met un champs de texte voici ce que cela m'affiche :

et quand je désactive la fonction création sa donne ça :

pouvez vous m'expliquer ? pourquoi mon champ texte n'apparaît pas correctement ?
