MMS Configuration on iPhone [Free Mobile]

Good evening,

I've had a Free Mobile subscription for €15.90 per month for several months now. So it's an all-inclusive plan (SMS, MMS, Calls, internet).

Here is my question: How do I configure my iPhone 3G to send MMS?

Here is what is being asked: Access point name: __________
Username: I think I entered the correct one
Password: I think I entered the correct one
MMSC: ______________________
MMS Proxy: __________________
Max size of MMS: ___________
MMS UA Prof URL: _____________

I think I've said everything, I look forward to your responses!
(Ps: I looked on Google but the information I found doesn't work on my phone, that's why I'm asking you the question).

    In 2016, with iOS 9.3:
    To have MMS Free on iPhone, you need to change the automatic Free configuration in the settings:
    Settings > Cellular Data > Options > APN Name: mmsfree (instead of free).
