Reactivate a lycamobile sim card

Hello,
my SIM card is deactivated because I haven't used my phone for 4 months. I contacted customer service by phone; I was told to go to ***@*** but I can't find how to reactivate it.
Can you help me? Thank you.

      Thank you very much. I did what was indicated, but I am being asked for the last 4 digits of the ICCID... where can I find these numbers? I only have the SIM card holder where I see the PUK code and the barcode. I tried using the last digits of the barcode, but it didn't work.
