Probléme de formulaire avec Dreamweaver 8!!

Bonjour,
Alors voilà aprés deux semaines de recherches j'ai réussi à créer un formulaire d'insciption assez compliqué pour mon site commercial et ça marche assez bien je reçois les mails dans ma bite que j'ai configuré dans outlook le seul soucis c'est que je reçois pas toutes les informations incluses dans le formulaire j'ai fait le test moi même et j'ai rempli toutes les cases mais à chaque fois les infos que je reçois sont les m^mes et manquantes.
S'il vous plait à l'aide car j'ai beau cherché mais je ne trouve pas ce qui cloche?
