How to enter a PUK code on a mobile?

Hello,
My daughter has locked her phone by entering wrong PIN codes!
I have the PUK code, but I can't find a way to enter it. I've tried three different phones, and each time I can't figure out how to enter this damn PUK code!
The phone manuals say nothing!!!
I also can't find anything online about a procedure on the phone that would allow me to enter the PUK code.
Do you have a solution?

Actually, it depends on the phone being used:
On my daughter's Samsung CT-G3300K it says "PUK code blocked"
On my old Motorola WX295 I have "SIM card locked"
On my wife's MOTOROLA RAZR2 it's even different "USIM disabled"!
None of the phones offer to enter a PUK code (or anything else)!
Any idea how to get out of this?
My daughter "played" with the codes on her phone! After trying 3 PIN codes on her sister's phone, they ended up on the PUK code and tried about ten numbers!!! Result, SIM card blocked and needs to be replaced! This is for those who have only heard about it, we tested it for you and it works, you can really "kill" your SIM card permanently by entering several incorrect PUK codes! Well, I gotta go, I have ears to pull!!!
