Well, I'll summarize the response:
-The campaign can only be played alone (that's why it only shows one Wiimote on the back of the box)
-The zombie mode can be played with 2 on the same console (with the possibility to play this mode online still allowing for 2 players)
-To connect the second controller, in the multiplayer menu (right after it says "connecting to Nintendo Wi-Fi connection) or in the zombie menu, press A with the second controller. When controller 2 lights up, start the game and play!!!

I bought the game Call of Duty Black Ops on Wii for 59.99 euros
And with my brother, we play together on the same screen and the same console,
1. If there's only one controller, it's because the Cooperative mode is played solo
2. You can play together in Online mode by creating a profile
And you only need 2 profiles! Then connect controller 2 when you are logged in and it should work... -

- press the A button on the second controller

Well, you shouldn't complicate things with the Wiimote; it's impossible to play two players on the same console because the Wii simply isn't sophisticated enough, and the sensor can't recognize whether it's the pointer of controller 1 or that of controller 2.
