PC crash with green screen

Hello, it's been 3 weeks now that my PC has been crashing. The first week there were some bugs here and there, and now this week my PC crashes on any game with varying play times (between 5 and 25 minutes). At first, it was just the game crashing, so I would go to the task manager and close the game in question, but now since today 06/14/17, my PC crashes completely, but only while gaming. I have no access to anything; I get a khaki green screen, the PC is running but unresponsive, I can’t see my mouse or anything. Please help before I end up formatting it..

My config:

Graphics card: Nvidia GeForce GTX 960
Processor: Intel i5 4570 / 3.2GHz
RAM: 16GB (replaced yesterday 06/13/17)
Motherboard: Asrock H87M
Cooler: Cooler Master Hyper 212 EVO
PSU: Zalman ZM500-GS
Coolermaster Cooler Master Hyper 212 EVO

Hello,
I think it's coming from the graphics card (I can't see where else it could be coming from)

download whocrashed (analyze and then dump tab) to be sure of where it's coming from, but normally it's the graphics card.
