    So for the first question:
Following the fact that Byakuya has taken back Rukia's powers, Ichigo finds himself undergoing training (led by Urahara) to "awaken" his powers that are deep within his soul (or something like that). Anyway, during this training, Urahara cuts Ichigo's chain of Karma. The chain starts to erode, and if it erodes completely, Ichigo will become a hollow. However, if he manages to get out of the hole before it completely erodes, he will become a shinigami. If you have closely watched the episodes or read the chapters regarding this part, you will see that when the chain reaches the end (there should be one link left), Ichigo is in the process of transforming into a hollow. But he resists not to transform while at the same time having a conversation with Zangetsu. The rest you know, he comes out of the hole with a hollow mask and wearing the uniform of the shinigami.
In short, I think his hollow appeared at the moment he was transforming.

    For the second question:
Tite Kubo is a big fan of the band Nirvana, and Bleach is their first album, which is kind of a shame.

There you go, I hope I have been clear enough for your first question and that I have helped you =)

It's because of the training to regain his Shinigami powers that he got this Hollow inside him, as he was transforming into a Hollow but fought against the transformation (well, that's what I think from the anime, but I believe it's the same). By the way, he pierces his own Hollow mask. And this mask will follow him everywhere because it's a part of himself (and this is just a hypothesis, right?) and the part of him he battles in the world of his Zanpakuto to be able to continue fighting Kenpachi Zaraki is his Hollow side. Well, there you go, I hope I answered your question!

Due to the training of Kisuke Urahara, the former captain of the 12th Division and the creator of the Research and Development Institute.

It's after Byakuya broke his powers with a technique, and Urahara gave him "special" training, that he broke the chain linking him to his body while Ichigo was out of his body.
Afterwards, the chain corroded, and he turned into a Hollow, and he struggled desperately and came out of it, that's it.

I also want to know why he has a hollow inside him; you say it's because of the special training, but every time all his enemies say, "Who are you? You're not human!" So I also think he is not human because if you look back at the early episodes of Bleach, Rikia says she has never seen a human with such great spiritual power, and when she wanted to give just half of her powers to Ichigo, he took all of her powers. Also, when he fully transforms into a hollow to protect Inoue and uses a cero, his enemy says that humans wouldn't be able to create a true cero. So I think he is not human but something else between humans, hollows, and shinigami.
      There you go, kisses.

épisode 296 Aizen tells him that he has done research on Ichigo, just like for the Vizards. That's why he has a Hollow, Ichigo has a Shinigami father and a human mother, which makes him different and therefore stronger. Aizen, thanks to the Hōgyoku, can create Vizards, Arrancars, and Espadas.

This is normal, Ichigo is indeed human, he has a great spiritual energy because his father is also a Shinigami.

What amazes me is that Ichigo is the only one who didn't need the Destruction Pearl to become a Vizard. Maybe he isn't entirely human.

Indeed, he is not entirely human; in the last chapter, Ichigo's father reveals to him that his mother was a Quincy, hence this incredible power.

Bleach means detergent in English. Regarding Ichigo's hair color, orange is strange in Japan, I think.
