Problème de script perl

Bonjour,
j'ai fait ne balise <form> et j'ai mais l'action (cgi-bin/form2mail.pl) un script qui envoie le contenu d'une formulaire à un e-mail, et s'affiche une erreur comme suivent, que je pas pu la résoudre depuis 3 jours.
je serais heureux de vouloir me répondre.
