This would imply that 3 incorrect attempts to enter the PIN code have been made??? And now a PUK code is required, is that what this is about???

It’s very strange to have “forgotten” this code which is nothing but the SIM card's PIN code; if this phone is indeed yours and the SIM card is indeed yours, there should be no reason to have forgotten this code since this SIM card should be yours.
