    Here’s what PlayStation says:

    If your service activation code is printed on a receipt, please contact your retailer to retrieve the code.

    For all other types of damaged codes, please contact us using this form, mentioning the purchase date and retailer. You may need to provide proof of purchase and photos of both sides of the code.
