In fact, this error code is commonly seen with modded consoles.
It comes from your hard drive. And the recovery mode can't fix this issue.
You'll need to reset it with a PC.
• remove the hard drive
• connect the hard drive to the PC (under Windows 7 64-bit, but any Windows operating system should work without additional software)
• open disk management - in Win7, it prompted me to create a primary boot record, so I did that.
• format NTFS - that's right, NTFS - do not use quick format
• turn off the PC, remove the hard drive, also take the USB drive
• put the hard drive back in the PS3, insert the USB stick into the PS3 (Don't forget to download the latest firmware from SONY onto the USB drive.)
• start PS3
• a screen appears indicating that the hard drive needs to be formatted
• hold Start + Select for 5 seconds.
• After formatting, the system will restart and the update will begin.
• set up your PS3 configuration - done
This will delete your data and backups but you have a chance to recover your console.
