Create a checkbox in open office

Hello,
I want to create a checkbox in Open Office but I don't know how to do it. I've searched but I couldn't find anything. Can you please help me? Thank you.

    Show form control toolbar

    Activate draft mode by clicking on the square and
    Select checkbox
