Unable to disable call forwarding on iPhone 4 iOs6

Hello,
I set up call forwarding to my work, and since then, I can't cancel it.
I have an iPhone 4 with iOS 6.1.2, I have turned it off several times, I performed a restore, removed the SIM card, tried through the menu, and also with universal codes (*21*, #213, etc.). Nothing works.
The icon is present (next to 3G), but when I go into "call forwarding," it seems disabled. If I activate it, I can't enter a number; the field stays empty, and if I try to disable it, there’s an error message. (The setting failed).
If I just dial #, I get "request execution error. No network service."
What can I do to resolve this issue?

When call forwarding is activated, a request is sent to the carrier. The error "no network service" implies that the iPhone is unable to communicate with your carrier to send the request to disable call forwarding.
1) If the problem lies with your iPhone, put your SIM card in another phone to disable forwarding.
2) If the issue persists, it may be due to your SIM card, your phone plan, or the carrier. In all these cases, calling customer service seems like an easy solution.
